Day ShiftDescription:Position Summary:Misidentification and/or an overlay of the patient’s record to reflect another patient’s identity, as well as incorrect information in a patient's chart leads to reductions in medical data integrity, reimbursement constraints, administrative inefficiencies, medical legal liability, compromised delivery of care and exposure(s) to patient safety.
The Chart Integrity Analyst (CI Analyst) performs a variety of activities to identify, investigate, and resolve wrong encounters and possible health record documentation errors in the electronic health record, both acute and ambulatory (physician practice) environments. The CI Analyst works closely with members of the TogetherCare HIM Operations Team, Data Integrity Liaisons, Health Informatics, clinicians, and ancillary departments to resolve errors; maintain the integrity of the patient record and ensure that all records are complete, accurate and unique to one patient. The chart correction scope includes collaboration with Registration, Billing, clinicians/clinical service lines, TogetherCare application analysts, Identity, and ambulatory (physician practices) to resolve documentation errors due to misidentification and/or an overlay of the patient’s record.
This position supports a function that must provide 24 hours per day, 7 days per week coverage for all of Trinity Health, including weekend and holiday rotations. It requires intensive focus with the ability to move multiple corrections and solutions through as quickly and accurately as possible. This is a remote 2nd shift position.
ESSENTIAL FUNCTIONSKnows, understands, incorporates, and demonstrates the Trinity Health Mission, Vision and Values in behaviors, practices and decisions.Manages Epic Chart Correction InBasket for ambulatory and acute settings; resolving Chart Correction submissions and opening/coordinating Chart Correction Cases in Epic, including wrong patient registered chart clean up.
Reviews and forwards MyChart Patient Advice InBasket messages and manages MyChart patient requests, working with the Data Integrity Liaisons and appropriate departments depending on the nature of the request.
Reviews potential chart correction cases and patient overlays/overlaps identified by users of the system through varied modalities, including Chart Correction submission forms in Epic, email and Service Now tickets.
Reviews and resolves complex and numerous Chart Correction Tasks associated with contact moves (due to wrong patients registered) and unmerges (due to erroneous merges) in Epic, which can span over multiple visits, working with clinicians and ancillary departments to complete said tasks. Identifies root causes of chart errors and their effects on the patient record including billing, compliance, and multiple downstream systems.
Updates records going through the chart correction process with the appropriate patient highlights; alerting users of potential errors to the record through flags and advisories. Ensures corrections are completed in a timely, accurate manner to support patient safety and accurate electronic databases.
Collaborates with the Data Integrity Liaisons and the TogetherCare Identity Team to perform thorough final reviews of patient records within all clinical and registration systems.
Directs users to appropriate avenues if requests are not Chart Correction related, i.e. registration errors, billing errors, coding queries, etc.
Provides users with Tip Sheets to guide them on how to make corrections that are submitted, but are the responsibility of the user to perform.
Test end user corrections within Epic Ministry Support (MSUP) to accurately provide guidance to users when self-correction is warranted.
Utilizes the "Mark Patients for Merge" activity for any patients that are identified; documents commentary to support decision for patient merge. Marks patients Non-Known Duplicates as appropriate.
Documents all actions taken to correct errors and the associated correspondence within chart correction cases and/or Contact Move Tasks. Validates correction of the error and resolves all chart correction cases and associated tasks before closing.
Escalates to leadership regarding the colleague who created the error to support ongoing training and education.
Reviews billing requests sent to the Chart Correction InBasket or via Service Now, and submits to the Billing Queues using appropriate Account Activities.
Rotates working customer call center and transcription pending to ensure timely completion of transcribed documentation as needed, while performing chart corrections and responding to Service Now tickets.
Provides assistance with Epic Incoming Messages from outside entities during off-shift hours to ensure that messages are accurately matched with patients and routed to providers in a timely manner.
Attends team and departmental meetings, as well as Mid-Revenue Cycle Town Halls.
Responsible for compliance with Organizational Integrity through raising questions and promptly reporting actual or potential wrongdoing.
Performs other duties as assigned.
Maintains a working knowledge of applicable Federal, State and local laws and regulations, Trinity Health's Integrity and Compliance Program, and Code of Conduct, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical, and professional behavior.
(Pay Range: $24.5303-$36.7954)
Minimum QualificationsEducation: High School diploma or GED required. Associates Degree in Health Information Management, Computer Science Informatics, Information Technology, or related equivalent field preferred.
Experience: Minimum five (5) years of extensive healthcare documentation and/or HIM experience with acute, ambulatory and/or physician office settings, with demonstrated knowledge of analysis methodology; and project and work planning, with increasing responsibilities.
Attention to details, advanced problem solving and high analytical skills required. Ability to analyze complex information and use problem solving skills to determine appropriate outcomes. Ability to make effective, timely decisions based upon sound judgement and data analytics, considering implications and consequences.
Ability to work independently with minimal supervision in a remote setting using electronic references.
Advanced computing skills required, Microsoft Windows, web applications and other electronic resources. Knowledge of hospital computer systems and EHRs required.
Knowledge of clinical workflow, medical record content, registration and billing is required.
Excellent interpersonal skills, high degree of initiative and ability to communicate effectively with all levels of the organization; especially medical staff, clinic service line leadership, Patient Financial Services, Revenue Integrity, Billing Services, Registration, and HIM to obtain compliance and resolution of chart integrity issues.
Ability to interpret and apply Joint Commission standards and HIPAA requirements to Document Creation and Management operations required.
Demonstrated ability to multi-task including changing priorities/focus quickly.
Able to accommodate a flexible, remote work schedule that supports a 24/7 operation.
