FlexStaff- Dietitian Specialist- TEMP Position

FlexStaff is seeking a dedicated Dietitian to join our client’s team on a temporary basis, providing comprehensive nutrition assessments, treatment, and coordination for PACE participants. As a key member of our interdisciplinary team, you will conduct individual and group nutrition assessments, develop personalized care plans, and provide essential counseling to support our participants’ health goals. Your work will ensure that each participant receives tailored nutritional support, promotes independence, and enhances their quality of life.

Responsibilities:

• Perform comprehensive assessments via face-to-face and telehealth consultations
• Develop and monitor individualized nutrition care plans
• Provide counseling and education to participants and caregivers
• Evaluate nutritional needs for home-delivered meals and supplements
• Collaborate with team members to adjust care plans based on participant progress
• Support community outreach and educational activities
• Maintain detailed documentation in compliance with agency standards
• Conduct home visits, including navigating diverse environments and working with caregivers

Qualifications:

• Bachelor’s Degree in Dietetics from an accredited program
• Current registration as a Registered Dietitian (RDN) with the Commission on Dietetic Registration
• New York State Certified Dietitian Nutritionist (CDN) license
• Minimum of 1 year of clinical nutrition experience, preferably with elderly or frail populations
• Home care experience is a plus
• Strong problem-solving, adaptability, and communication skills
• Ability to interpret professional and regulatory materials
• Legally authorized to practice in New York and up to date on immunizations
