UPMC Presbyterian is hiring a Full-Time Diabetes Educator to support the Internal General Medicine Department. This Monday–Friday daylight position includes travel to our West Mifflin office one day per week.
Purpose:
The Diabetes Educator helps patients understand and manage diabetes in a primary care setting. This role focuses on empowering patients with the knowledge and tools they need for long-term self-management and healthier outcomes.
You’ll work closely with providers to reinforce care plans, support lifestyle changes, and reduce the risk of complications through education and ongoing follow-up.
Responsibilities:
Patient Education and Support
Provide diabetes education based on National Standards for Diabetes Self-Management Education
Teach patients and families about:
Blood glucose monitoring
Medication and insulin administration
Insulin pumps
Nutrition and healthy lifestyle habits
Risk reduction and complication prevention
Care Planning and Follow-Up
Assess patient learning needs and create individualized education plans
Review home glucose logs and medication regimens
Monitor glycemic control and answer patient questions
Adjust insulin or medication doses using approved protocols (when appropriate and in collaboration with providers)
Program & Team Collaboration
Support efforts to achieve and maintain American Diabetes Association (ADA) Recognition
Provide diabetes education updates and training to clinical staff
Contribute to quality improvement initiatives for chronic disease management
Licensure, Certifications, and Clearances:
*Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
