About This Role:
We are seeking qualified applicants to fill a full-time Therapist II position in the New Horizons Case Management Program. The New Horizons Case Management (NH CM) Program offers a team environment and flexible work hours.

Job Responsibilities Include:
•Maintaining a caseload of case management clients, to include Family Assessment Planning Team (FAPT clients.
•Providing effective mental health, substance use and co-occurring case management services to youth and their families.
•Providing crisis intervention for clients, evaluating risk and need for hospitalization.
•Conducting comprehensive assessments, including ASAM Criteria and DLA-20, formulating diagnoses, and making recommendations for level of care treatment needed.
•Developing and implementing treatment plans to promote progress of youth served.
•Timely clinical documentation into an Electronic Health Record that meets Medicaid, state, local and agency documentation standards/expectations.
•Documentation of all services, contacts and attempted contacts with youth, their families and collaterals.
•Collaborating with other departments, professionals, and other stakeholders in the community (i.e., school staff, probation officers, psychiatrists, social workers, private providers, and residential treatment staff).
•Maintaining healthy boundaries with clients and their families.

Minimum Requirements:

•Master’s Degree in psychology, social work, counseling, or related behavioral science field
•Ability to demonstrate licensure eligibility for professional licensure as LCSW or LPC in the Commonwealth of Virginia
•1-2 years’ experience providing services to children, adolescents and families with issues related to substance use, mental health and co-occurring disorders.

Preferences:
•Experience conducting youth mental health, substance use and co-occurring assessments, to include the ASAM Criteria, DLA-20, formulating diagnoses and developing treatment plans for varying populations and complex clients and families.
•Experience providing youth mental health, substance use and co-occurring case management services.
•Experience providing crisis intervention for clients, evaluating risk and need for hospitalization.
•Experience working with and presenting cases to multidisciplinary teams.
•Experience working with Medicaid-funded services for youth.
•Experience providing services in home and community-based settings.
•Experience collaborating with other service providers.
•Bilingual and multilingual candidates encouraged to apply

Special Requirements:
•Must be eligible for professional licensure as LCP, LPC or LCSW in VA
•Must possess a valid driver's license
•Must pass DMV record check
•Must pass TB test
•Offer of employment is contingent upon the candidate passing a fingerprint based National Criminal History Record Check
•Child Protective Services Requirements: Effective 1999, mandated by the Department of Behavioral Health and Developmental Services 12VA35-105-400 rules and Regulations for Licensing Providers by the Department of Behavioral Health and Development Services related to Criminal Registry Checks, a search of the registry of founded complaints of child abuse and neglected maintained by the Department of Social Services.

Work Schedule:
This full-time position is a community-based position (visiting homes and locations within the community providing case management) and is primarily based at our Woodbridge office. The position requires a 4- or 5-day work week that includes evening hours.

Hiring Salary Range:$76,069.50 - $90,372.75

This position qualifies for a hiring bonus of $5,000. This bonus will be paid in two equal installments. The first installment of $2,500 will be paid after you have completed 90 days of employment. The final $2,500 installment will be disbursed upon successful completion of the probationary period.
