Location: This position is based in Lafayette, with supporting staff in Bloomington and Muncie
Responsibilities:
This position plays a crucial role in HIV Medical Case Management, which is the delivery of client-centered activities focused on improving health outcomes in support of HIV care continuum.
Medical Case Management includes all types of case management encounters (face to face interactions, phone contact, and any other forms of communication).
The Medical Case Manager will work closely with our Linkage to Care Case Manager in addition to infectious disease providers, other local HIV service providers and community partners.
Collaborate with healthcare and community partners to improve health outcomes
Supports the development and execution of community outreach strategies, initiatives and events that align with IU Health community outreach priorities, in meeting the identified needs of the communities.
Provides assistance to team member volunteer programs.
Provides administrative coordination for the system team in areas such as meeting management, budget coordination, and ad hoc project support.
Supports the assigned Manager and team in successfully executing initiatives and events.
Qualifications:
Bachelor's degree or equivalent years of experience required.
0-3 Years of Experience.
Ability to work in a complex business environment.
Ability to manage multiple projects and deadlines, background managing program/event logistics.
Experience in managing program/event logistics preferred.
