Overview

CVP seeks Registered Nurses to join our team in providing our Nation's Veterans with skilled nursing and rehabilitation services at the Louis Stokes Cleveland VA Medical Center (LSCVAMC) in Cleveland, OH. CLC RNs will serve across a diverse array of specialized neighborhoods including short-stay rehabilitation, respiratory-focused post-acute care, complex wound care and IV antibiotic management, cognitive impairment and dementia care, blind rehabilitation, and inpatient hospice care.

This role requires a compassionate, skilled clinician who can adapt to the unique needs of each neighborhood while delivering high-quality, patient-centered care in a homelike long-term care environment. Shifts are primarily 12 hours, with some 8-hour and 10-hour shifts available. Day, evening, night, weekend, and holiday availability is required.

Responsibilities

  • Perform comprehensive patient assessments and develop, implement, and update individualized plans of care in collaboration with an interdisciplinary team.
  • Administer medications, including IV therapy, and manage central and peripheral line IV medications in accordance with facility policy and established standards of care.
  • Perform tube feedings, Foley catheter care, wound care (simple to complex), tracheostomy care, and other skilled nursing interventions as required by patient condition.
  • Draw labs and obtain and deliver specimens as directed.
  • Assist patients with activities of daily living (ADLs) and support rehabilitation goals aimed at restoring prior functional status and facilitating safe discharge to the Veteran's preferred setting.
  • Coordinate admissions, discharges, internal and external transfers, and discharge planning in collaboration with the interdisciplinary care team.
  • Delegate appropriately to assistive personnel, including Nursing Assistants and Health Technicians, while maintaining accountability for overall patient care.
  • Document all patient care accurately and completely in the Electronic Health Record (EHR) prior to the end of each shift.
  • Monitor and respond to changes in patient condition, notifying the assigned physician or charge nurse as appropriate and revising the plan of care accordingly.
  • Participate in interdisciplinary care planning meetings and collaborate with physical, occupational, and speech therapy teams to support rehabilitation and recovery goals.
  • Provide holistic, patient-centered care to Veterans enrolled in hospice services, including bereavement support for families, in accordance with the Heroes Harbor Hospice neighborhood model of care.
  • Provide specialized care to Veterans with cognitive impairment and dementia, applying evidence-based approaches to behavioral management and safety.
  • Float between CLC neighborhoods as directed by the VA Nurse Manager or designee based on operational and staffing needs.
  • Participate in continuous quality improvement activities, Unit Practice Council (UPC) meetings, Nursing Quality Council, Professional Practice Council, unit staff meetings, safety huddles, and bedside reporting as required.
  • Report all patient safety incidents to the Contracting Officer's Representative (COR) within 24 hours in accordance with VA patient safety policy.
  • Maintain compliance with all VA, Joint Commission, and facility-specific policies, procedures, and standards of conduct.

Qualifications

  • Must be eligible to obtain a Public Trust government security clearance; U.S. Citizenship is required.
  • Graduate of a school of professional nursing approved by the appropriate State agency and accredited by the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE).
  • Current, full, active, and unrestricted Registered Nurse license from the Ohio Board of Nursing or a current multi-state license from a Nurse Licensure Compact (NLC) state.
  • Minimum of two (2) years of direct patient care experience within the last three (3) years; experience in long-term care, skilled nursing, or rehabilitation nursing is strongly preferred.
  • Current Basic Life Support (BLS) certification from the American Heart Association or equivalent accredited certifying body.
  • Current CPR certification from an American Heart Association vendor.
  • Ability to speak, read, write, and understand English fluently.
  • VA experience is preferred.
  • Specialty certification in gerontological nursing (RN-BC), rehabilitation nursing (CRRN), or hospice and palliative care nursing (CHPN) is preferred.
  • Proficiency in Electronic Health Record (EHR) documentation; experience with VA VISTA/CPRS systems is a plus.
