The Behavioral Health Clinical Therapist is responsible for conducting sessions with patients to complete thorough assessments and evaluations, diagnosis and develop treatment plans, and provide therapy services, which may include brief intervention, supportive counseling, crisis intervention, or long-term psychotherapy. The Behavioral Health Clinical Therapist may provide therapy services to inpatients and/or outpatients on an individual, couples, family, or group basis, and may serve populations with a variety of complex clinical needs. The Behavioral Health Clinical Therapist collaborates and consults with interdisciplinary treatment teams as appropriate.
This Clinical Social Worker position will support programs within the UVA Health – Population Health Department. Our programs address the increased prevalence of chronic disease management, patient access to primary and specialty care, and care delivery beyond hospital walls. Population Health programs aim to lower healthcare costs for patients and systems by preventing readmissions and non-emergent ED visits, decreasing patient length of stay, and ensuring continuity of care once patients are discharged from an inpatient stay in our Medical Center or Emergency Department, Ambulatory Clinic, Children’s Hospital, or by direct provider referral.

The Population Health Department’s Behavioral Health Team is seeking a dedicated, compassionate Licensed Clinical Social Worker to join the team. This full-time, hybrid role will primarily focus on providing psychotherapy services to program patients, including the Medicine HOME Program - a complex care program whose purpose is to improve consistency and quality of care through individualized care plans, clinical coordination, and mitigation of social drivers of health (SDOH) risks for frequently hospitalized patients. These patients have complex medical, behavioral, and social needs that require intense interdisciplinary collaboration across care teams.

This Licensed Clinical Social Worker role will require credentialing in order to bill for services on a templated schedule, Monday – Friday. 20 hours per week. Hybrid position.

  • Provides evidence-based, person-centered clinical therapeutic interventions around a broad range of behavioral health conditions.
  • Completes bio-psychosocial assessments, diagnosis, and development of culturally competent individualized treatment plans for patients in both individual and group settings.
  • Participates in treatment planning, care coordination, and advocates for patient preferences within the interdisciplinary team, when appropriate. Collaborate with patients (and care partners, when appropriate) to create, follow and maintain progress on treatment plans.
  • Documents patient care in accordance with UVA University Medical Center policies and regulatory requirements.
  • Facilitates staff development, in-service training, and educational opportunities to support professional growth and evidence-based practice.
  • Provides clinical supervision, and trains new staff, students, and interns as assigned.
  • Assists with treatment program development and engages in research activities.

MINIMUM REQUIREMENTS

Education: Master’s degree in Social Work from a CSWE accredited social work program required

Experience: 3 years’ experience post master’s required.

Licensure: Licensed to practice as a Licensed Clinical Social Worker in the Commonwealth of Virginia (LCSW) or applicants with an active license from another state must obtain Virginia LCSW within 18 months of hire date.

PHYSICAL DEMANDS

Job requires sitting for prolonged periods, standing/traveling or use of assistive and climbing (stairs, steps). Proficient communicative, auditory and visual skills; Attention to detail, hear, speak, see, distinguish colors, read, ability to write legibly; Ability to lift/push/pull less than 20lbs. May be exposed to chemicals, blood/body fluids and infectious disease.

The starting base rate for this role is $29.54 hourly. Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education.
