Short Description
The Clinical Pharmacy Specialist provides advanced clinical pharmacy services in medication management to ensure optimal safety and cost-effective therapeutic regimens through Collaborative Practice Agreement. (CPA).
The functions include but are not limited to:
- Provides comprehensive direct patient care via pharmacist/provider CPA including implementing, modifying and discontinuing medications; collecting, analyzing, and monitoring of patient data and ordering laboratory tests.
- Manages referred patients for disease states in gastroenterology and rheumatology to improve clinical outcomes, patient safety, and cost-effective care.
- Provides clinical pharmacy services for patients, including patient counseling, selection of appropriate therapies, monitoring parameters and frequency of monitoring, management of adverse effects and identification/management of drug-drug interactions.
- Assist with medication access, including prior authorizations, appeal letters and peer to peer reviews.
- Identify and complete patient care quality improvement projects.
- Educate health professionals and patients through in services and written/verbal materials.
- Provides preceptorship and mentorship to students, faculty and residents.
- Multidisciplinary collaboration for policies, procedures, guidelines and protocols.
- Provides input and supports pharmacy department initiatives in the daily collaboration with the clinical teams.
- Effectively communicates recommendations and education to patients, pharmacy and the healthcare team.

Experience Required
- 3-5 years Ambulatory experience preferred.
Education Requirements
- Pharmacy Doctorate Degree
License/Certification Requirements
- New Jersey Board of Pharmacy Licensure.
Special Requirements
- Postgraduate (PGY2) Residency in Ambulatory Care Pharmacy/Medication Management/Pharmaco Therapy Required.
- Board of Pharmacy Specialties (BPS) Certification Ambulatory Care Pharmacy (BCACP) (or eligible within 2 years).
