Description of Responsibilities

The Certified Home Health Aide (CHHA) is responsible for providing personal care and supportive services to patients in the home setting under the supervision of a Registered Nurse or Therapist. This role supports the organization’s commitment to delivering high-quality, compassionate home health services by assisting patients with activities of daily living, monitoring and reporting patient conditions, and maintaining a safe and supportive care environment. This hybrid position also provides administrative support to the Home Health department during periods of low census, including coordination of inter-team communication, chart audits, faxing and receiving physician orders, and management of email and collaboration platform communications. The CHHA is responsible for completing assigned patient care and administrative duties in compliance with agency policies, regulatory requirements, and quality standards.

Responsibilities include the following:

1. Providing hands-on personal care including but not limited to:

-Baths

-Oral hygiene

-Assisting patients with dressing and undressing

-Skin care to prevent breakdown

-Assisting the patient with toileting activities

-Keeping patient’s living area clean and orderly, as appropriate

2. Planning and preparing nutritious meals.

3. Assisting in feeding the patient, if necessary.

4. Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered.

5. Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency).

6. Reporting on patient’s condition and significant changes to the assigned nurse.


Minimum Qualifications:

• Current experience providing patient care in a home health, healthcare, or caregiving setting preferred.

• One (1) year of Home Health experience preferred.

• Previous administrative or clerical experience in a healthcare setting preferred.

• One (1) year of experience with EMR systems preferred; CareTend, KanTime, or CPR+ experience preferred.

• Strong verbal and written communication, organizational, and documentation skills required.


Education and/or Experience:

• Current Certified Home Health Aide (CHHA) certification in the State of California required.

• Current BLS/CPR certification required.

• Valid California driver’s license, reliable transportation, and current automobile insurance required.
