Reporting to the Medical Director, the Registered Dietitian/Certified Diabetes Care and Education Specialist (RD/CDCES) will serve as an essential clinician within our Cardiometabolic Center — an innovative, interdisciplinary care model that brings endocrinology, cardiology, nephrology, and ancillary services under one roof to deliver integrated, wrap-around care for patients with complex cardio-kidney-metabolic disease.
The RD/CDCES will be a strategic partner in reshaping how chronic cardiometabolic disease is managed, working alongside physicians, advanced practice providers, behavioral health, and care management teams to drive both optimized medical management and the sustainable lifestyle change that determines long-term outcomes. Core responsibilities include delivering evidence-based diabetes self-management education and support (DSMES), providing medical nutrition therapy and lifestyle counseling, and developing patient education materials and programming that embed empowerment and partnership into every encounter.

Responsibilities
Direct Patient Care & Education
- Provide individualized and group-based diabetes self-management education and support (DSMES) in accordance with American Diabetes Association standards
- Provide medical nutrition therapy (MNT) and nutrition counseling tailored to the cardio-kidney-metabolic patient population
- Assess patient readiness to learn, health literacy, cultural needs, and barriers to care
- Develop and implement personalized care plans addressing nutrition, medication adherence, glucose monitoring, blood pressure and lipid management, and lifestyle modification
- Educate patients on insulin administration, oral and injectable medications, continuous glucose monitors (CGMs), and glucose monitoring devices
- Build longitudinal relationships with patients, serving as a consistent point of contact and trusted partner throughout their care journey
Interdisciplinary Collaboration
- Collaborate closely with endocrinology, cardiology, nephrology, advanced practice providers, behavioral health, and care management teams to optimize treatment plans, reinforce shared care goals, and escalate care when appropriate
- Participate in care team huddles, case conferences, and quality improvement initiatives
- Serve as a knowledge resource for colleagues, staying current with evidence-based guidelines across diabetes, obesity, cardiovascular, and kidney disease management and helping disseminate best practices across the care team
Program Development & Quality
- Develop, curate, and continuously improve patient education materials and curricula tailored to diverse health literacy levels and cultural backgrounds
- Design and lead group education programs, shared medical appointments, and cohort-based lifestyle change programming
- Contribute to the development of clinical pathways, workflows, and protocols that integrate education and lifestyle intervention into the Center's care model
- Monitor and track clinical outcomes including A1c, weight, blood pressure, lipid profile, renal function, and adherence metrics
- Support population health initiatives and quality metrics targeting cardiometabolic disease control
- Assist with patient outreach, education programs, and community health initiatives
- Document all patient encounters accurately in the electronic medical record
Qualifications
- Active Registered Dietitian (RD) license in New York State
- Current Certified Diabetes Care and Education Specialist (CDCES) certification (formerly CDE)
- Bachelor’s or Master’s degree in Nutrition, or related field preferred
- Minimum 2–3 years of experience in diabetes education, chronic disease management, or outpatient clinical care
- Experience working in an FQHC, community health center, or ambulatory care setting preferred
