As a Case Manager familiar with the aging process and service delivery to older adults and disabled adults, you will assist clients in the Health Home Program by providing care coordination, helping individuals connect with and navigate various healthcare and social services. You will work with eligible individuals to identify health goals and available community services to meet health needs and monitor both the progress of the individual and the adequacy of the services being provided. You will be out in the community visiting with your clients each month.

How to be Successful in the Role:
We are looking for someone with case management experience and knowledge of person-centered care. Someone who can assess client needs and make individualized recommendations in partnership with their family and other support individuals. You will be diligent and detail-oriented with documentation and can write in a clear and concise manner. You will communicate across various social, cultural, educational, and economic backgrounds, so you should be able to adapt quickly based on the audience.
