Job Title: Care Manager – Emergency Department (ED)

Schedule: Monday – Friday

Shift: 9:00 AM – 5:00 PM

Hours Per Week: 35 hours

Contract Duration: 13 weeks (possible extension)

Compensation Details

Travel Pay Package

• Weekly Gross Pay Approx: $2,430 (Approx.)
• Weekly Stipend Approx: $1,730

Local Pay Option

• Weekly Gross Pay Approx: $2,100 (Approx.)
• Hourly Gross Pay Approx: $60/hour

Job Summary

The Care Manager – Emergency Department (ED) is responsible for coordinating patient-centered care across the healthcare continuum. This role focuses on care coordination, utilization review, discharge planning, and addressing patient clinical and psychosocial needs.

The Care Manager works collaboratively with interdisciplinary teams to improve patient outcomes, reduce hospital readmissions, and ensure appropriate resource utilization.

Skills & Experience Requirements

• Minimum 5 years clinical experience as a Registered Nurse required
• Experience in Care Coordination, Utilization Review, and Discharge Planning required
• Leadership experience preferred
• Strong computer literacy required
• Experience addressing social determinants of health preferred

Education Requirements

• Bachelor’s Degree required
• Master’s Degree preferred
• Bilingual Spanish preferred

Required Certifications & Licenses

• Active New York State RN License required
• PRI and Screen Certification required within 3 months of employment
• Certification in Case Management preferred
