RN Case Manager (Hybrid)Location: Washington, DC 20007Schedule: Monday – Friday, 8:00 AM – 5:00 PM (4 days onsite, 1 day remote)Pay Rate: $47–$49/hour Position OverviewSeeking an RN Case Manager to support a complex patient population with chronic conditions such as diabetes, hypertension, and COPD. This role focuses on care coordination, discharge follow-up, and ongoing patient engagement while ensuring compliance with NCQA standards. Key ResponsibilitiesManage a caseload of up to 45 patients with high-acuity needsConduct telephonic clinical assessments and bi-weekly patient outreachCoordinate care across providers, specialists, and community resourcesSupport hospital discharges and ensure safe transitions of careDevelop and manage individualized care plans in line with NCQA guidelinesIdentify care gaps and connect patients to necessary medical and social servicesCollaborate with interdisciplinary teams and participate in case reviews Required QualificationsActive DC RN License (required at time of hire)Minimum 2+ years of nursing experience1–2 years of case management experienceExperience working with high-acuity or frequently hospitalized patientsFamiliarity with NCQA standards and guidelines
