RN Case Manager (Hybrid)

Location: Washington, DC 20007

Schedule: Monday – Friday, 8:00 AM – 5:00 PM (4 days onsite, 1 day remote)

Pay Rate: $47–$49/hour

Position Overview

Seeking an RN Case Manager to support a complex patient population with chronic conditions such as diabetes, hypertension, and COPD. This role focuses on care coordination, discharge follow-up, and ongoing patient engagement while ensuring compliance with NCQA standards.

Key Responsibilities

• Manage a caseload of up to 45 patients with high-acuity needs
• Conduct telephonic clinical assessments and bi-weekly patient outreach
• Coordinate care across providers, specialists, and community resources
• Support hospital discharges and ensure safe transitions of care
• Develop and manage individualized care plans in line with NCQA guidelines
• Identify care gaps and connect patients to necessary medical and social services
• Collaborate with interdisciplinary teams and participate in case reviews

Required Qualifications

• Active DC RN License (required at time of hire)
• Minimum 2+ years of nursing experience
• 1–2 years of case management experience
• Experience working with high-acuity or frequently hospitalized patients
• Familiarity with NCQA standards and guidelines
