*Pool shifts are available on the PM and NOC Shifts, 2pm - 10:30pm and 10pm - 6:30am *Pool staff are required to work 4 weekend shifts per month, with additional opportunity to pickup *Pool staff are required to work either Thanksgiving or Christmas. *Pool requirements are year-round
Performs patient care under the direction of a registered nurse in compliance with nursing service policies and procedures. 95% - Provides care to assigned resident group.
Works as a team member to assure that all residents receive necessary care and services.
Immediately reports resident health concerns to the RN.
Responds to call lights of all residents in a timely manner.
Reports all allegations of abuse, neglect or misappropriation to the supervisor.
Assists residents at meal and snack times.
Assures that residents maintain adequate nutrition and hydration. Offers thickened liquids and therapeutic diets as needed.
Maintains resident safety during transfers and when using mechanical lifts, gait belts, etc.
Assures all resident supportive devices are applied in the proper manner.
Delivers restorative care specific to each resident. Follows the functional maintenance program.
Documents vital signs, weights, intake and output, observation flow sheets, restorative nursing and other assigned charting in a timely fashion as assigned by licensed nursing supervisors.
Performs assigned tasks in an emergency situation (ie. fire, tornado, disaster, etc.).
5% - Performs other duties as assigned.
May be "mandated" to stay past the normal shift in order to meet minimal staffing requirements.
Shows respect for residents, staff and visitors by accepting assignments willingly.
Has a customer service attitude. Uses a kind and gentle approach with residents while maintaining their dignity and respecting their privacy.
Maintains prompt and regular attendance. Reports to work as scheduled. Arrives in the report area at the shift start time. Leaves unit only for allowed break and meal times and returns within time frame established by RH policy.
Follows the Rock Haven dress code.
Organizes and completes assignments in a timely manner.
Carries out all facility policies.
Completes a minimum of 12 in-service hours annually.
Ability to communicate effectively orally and in writing.
Ability to effectively present information and respond to questions from managers, employees, residents, families, professionals and the general public
Ability to work well as a team member.
Completion of 120 hour nursing assistant course at an accredited program.
Long-term care experience preferred.
Special Requirement
Current certification with the Wisconsin Nurse Aide Registry.
Ability to sit, stand, bend, and stoop for periods of time.
Ability to lift up to 50 pounds occasionally.
Ability to work in a stressful environment.
Ability to respond to emergency/crisis situations.
