PUEBLO REGIONAL CENTER (PRC)Pueblo Regional Center (PRC) is one of three state-operated Regional Centers providing quality, specialized services and support to people with developmental disabilities and dual diagnosis by promoting growth, participation, contributions, and capabilities. PRC manages 9 group homes throughout the Pueblo West community, licensed to serve 68 people. PRC provides 24/7 care and support in a home-like setting, while offering habilitative services, activities of daily living and support in the areas of residential, Bridges, therapies, health and nutrition.About the Position

This position is essential in the coordination of Community Integration and Habilitation Services (Bridges) Employment First and Pre-Vocational (pre-voc) coordination for disabled people residing at PRC. Working with the Bridges Director, Employment Navigator (Empl Nav), and Employment Specialist (Empl Spec) to ensure Bridges Employment First and Pre-Vocational (Pre-voc) services and support are planned and scheduled to meet people's needs around employment. This position ensures employment programming is implemented in all designated Bridges employment and pre-voc areas. This includes providing supervision, modeling, and coaching to Empl Nav, Empl Spec, Bridges HCT III and direct support professionals. This position spends a portion of the work day outside the office working directly with people supported, employers, Division of Vocational Rehabilitation (DVR), Empl Nav, Empl Spec, ensuring the milieu of employment locations, and pre-voc locations are meeting the needs of people supported and run efficiently. This position works closely with the CIE Director to ensure implementation of services is occurring and meeting state and federal regulations. This position will be an integral part of the Employment First Initiative.

This position manages a section (multiple workrooms, employment and pre-voc sites) of Bridges services and directly oversees the Bridges Empl Nav, Empl Spec who in turn oversee the day to day running of Bridges services with the HCBS Waiver licenses in relation of employment and pre-voc. Responsibilities consist of overseeing the writing, implementation, monitoring of individual support plans (ISP) for learning and development of skills related to activities of daily living, employment and pre-voc skills, collaboration with teams in regards to IDT/MDT, quarterly meetings, and the writing of the annual service plan when is person is working with employment, pre-voc or DVR. This position engages with all internal departments, community partners/members, and external customers. Responsibilities also include scheduling when applicable to employment, pre-voc services and support.

The Health Care Technician IV (HCT IV) serves as a supervisory leader within the Bridges Program, responsible for overseeing daily operations, staff performance, and service delivery to ensure high-quality, person-centered care. This role provides direct supervision to HCT III staff and Direct Support Professionals, including coaching, scheduling, training, and performance management. The HCT IV ensures compliance with all regulatory requirements, supports implementation of individualized service plans, and maintains oversight of documentation, safety practices, and community-based programming. This position also collaborates with interdisciplinary teams, supports problem-solving in real time, and ensures services are delivered effectively, efficiently, and in alignment with program expectations.

Coordination:
  • Develops and coordinates appropriate Bridges supports and services for people in HCBS Waiver engaging in activities of daily living, Employment First and Pre-Vocational services.
  • Monitoring of documentation for Division of Vocational & Rehabilitation (DVR) services, Employment First and pre-vocational (pre-voc) services.
  • Ensuring compliance with federal regulations, state regulations, division policies and agency procedures.

  • Responsibilities also include scheduling of employment and pre-voc services, DVR meetings and meetings with external employers.
  • Ensures employment and pre-voc areas have work equipment and learning materials to meet people's needs according to the ISP and employment and pre-voc goals.
  • Responsibilities may include scheduling of community activities and activities run by Bridges department.
  • Support of community activities and activities run by the Bridges department may require change in hours and days worked.
  • Step in and take on the duties that the position oversees during periods of approved time off, injury on the job or illness. Ensuring ongoing support for people and preventing a break in skill building.Direct Care Services:
Supervision:
  • Exercise supervision over the Bridges Employment Navigator and Employment Specialist (approx. 3) staff.
  • Complete Performance Management Plans- Core competencies, goals and professional development.
  • Monitor staff training compliance and provide training when a need is identified.
  • Oversee timekeeping and leave approval via Kronos/UKG, When to Work (W2W) and Google Calendar.
  • Collaborate with the Community Engagement & Integration Director (CEI) to address performance issues.
  • Implements the overall operations of Bridges support and ensures regulatory compliance with federal and state regulations, division policies and agency procedures.
  • Oversee Bridges Employment Navigator and Employment Specialist staff to ensure caseloads needs are met in conjunction with implementation of ISP plans/programs and personal objectives.
  • Ensure Bridges Employment Navigator and Employment Specialist staff comply with CDHS Code of Conduct
  • Monitors, trains and observes Bridges Employment Navigator and Employment Specialist staff to ensure compliance with all departmental expectations.
  • Act as liaison between Bridges Employment Navigator and Employment Specialist staff, residential staff, department directors, and other entities internal and external to the agency.
  • Direct contact with people support daily either in a work environment or an educational setting to assist with learning of new skills and maintenance of already established skills.
Compliance with Agency Requirements:
  • Plans for certification/funding audits and allocates resources to meet annual and daily needs of Bridges Employment Navigator and Employment Specialist staff, sites and work locations. (This includes, but not limited to budget, staff scheduling, annual services for people served, transportation and supplies).
  • Identifies, evaluates, and ensures compliance to CMS & HCPF Rule, CDHS policies, DRC policies and agency procedures.
Administrative:
  • Collects, completes, monitors, and delegates administrative forms, reports and records in order to achieve ongoing licensure and certification.
  • Completes duties using prescribed electronic and hard copy procedures utilizing computers and other supporting tools as needed to complete the duties.
  • Overseeing the writing, implementation, monitoring of individual support plans (ISP) for learning and written contribution of data/identified issues in regards to IDT/MDT, quarterly meetings, writing of the annual service plan, employment and pre-voc outcome/goal development.
  • Assist people supported with submission of applications, vital documents, interview preparation, reviewing of job offers, signing of offer letters, completion of legal documents of employment, and electronic deposit as needed.
Training:
  • Participates in agency work groups, committees, attends assigned meetings and in-services as scheduled.
  • Obtain ACRE certification for Employment First employment and pre-voc services.
  • Annual recertification is required for “CORE”, delegated skills, and Safety Care.
  • Train Employment Navigator and Employment Specialist staff in departmental process, implementation of Bridges employment and pre-voc supports, assessment completion, quarterly/annual meeting documentation, and employment and pre-voc ISP program writing/monitoring.
  • Work directly with people support to learn employment skills from the most simplistic such as appropriate personal hygiene, appropriate attire for a job, learning computer skills, required vital document, money management, clocking in/out, using online/in person resources for potential employment, completion of a job application, time management, and more advanced skills that are specific to a job/required to maintain employment.
Other Duties as Assigned:
  • This position may be required to work at multiple locations and/or shifts as needed to address department and agency needs.
  • This position may be required to drive state vehicles with people who live at PRC. Must travel between group homes, Bridges sites and community locations to provide support and training. When not transporting people, this person may drive a State vehicle or choose to drive their own.
Special Qualifications:
  • Valid, current, and unrestricted Colorado LPT license or QMAP checked off by PRC Nursing is required for this position or must be able to earn QMAP within 3 months.

Education/Licensure/Certification/Experience:
  • High school diploma or General Education Diploma (GED)
  • Three (3) years of experience in an occupational field related to the work assigned to the position
  • Certification or licensure appropriate to the work assigned to the position

Substitutions

Additional appropriate education will substitute for the required experience on a year-for-year basis.


Preferred Qualifications:
  • 2 years of field experience and 1 year of supervisory
  • Experience as a mentor, trainer, or supervisor over healthcare staff.

  • Shift Work . PRC is a 24/7 facility, therefore person in this position must be available to work any shift, any day and/or any area, to meet agency needs.
  • Travel. Position must travel to remote work sites and must utilize a State vehicle, if available. If not must utilize a personal vehicle and submit for mileage reimbursement. Position is required to use a State
    vehicle when transporting individuals.
  • Special Qualifications. Valid, current, and unrestricted Colorado LPT license or QMAP checked off by PRC Nursing is required for this position or must be able to earn QMAP within 3 months.
  • Must demonstrate the ability to lift 50 lbs, bend, kneel, squat, etc, and perform interventions proficiently throughout employment.
