What You’ll Do

In this role, you will:

  • Provide individual, family, and group counseling
  • Deliver crisis intervention and behavioral health support
  • Complete mental health screenings, assessments, and treatment plans
  • Partner with primary care providers to address mental health and psychosocial needs
  • Support patients with behavior change, self-management skills, and lifestyle goals
  • Help identify barriers to care and connect patients with appropriate resources
  • Lead patient education and support groups
  • Collaborate with care team members to coordinate patient care
  • Maintain timely, accurate, and confidential documentation
  • Participate in peer review, case consultation, and quality improvement activities


What Makes You a Great Fit

  • Master’s degree in social work with limited or full licensure, or Licensed Professional Counselor licensure required
  • Minimum of 2 years of Social Work experience preferred
  • BLS certification required or ability to obtain
  • Strong understanding of behavioral medicine and evidence-based treatment approaches
  • Comfortable working in a collaborative, primary care setting
  • Strong assessment, communication, documentation, and time management skills
  • Commitment to patient confidentiality and compassionate care
