Job Title: Behavioral Health Care Coordinator

Location: Remote (Travel may be required)

Contract Duration : 3 Months

Work Hours: Monday to Friday | 8:00 AM – 5:00 PM

Key Responsibilities

• :Conduct health assessments and develop individualized care plan
• sCoordinate healthcare services, transitions of care, and community resource referral
• sEducate members on disease management, wellness programs, benefits, and available resource
• sMonitor member progress and update care plans as neede
• dCollaborate with providers, caregivers, and interdisciplinary team
• sAddress member inquiries and resolve care-related issue
• sMaintain accurate documentation and meet productivity standard
• sSupport audit readiness, quality initiatives, and special project
• sEnsure compliance with HIPAA, NCQA/URAC standards, and state regulation

Qualification

• s:Current, unrestricted RN, LPC, LMSW, LCPC, LCSW, LMFT, LPCC, LISW, LMHC, or equivalent license in the state of operation
• s.Minimum 2 years of direct clinical experien
• ce3+ years of managed care, wellness, case management, or care coordination experience preferr
• edStrong care planning, health education, and medical management knowled
• geExcellent communication, customer service, and relationship-building skil
• lsProficiency with Microsoft Office, databases, and web-based applicati
• onAbility to travel within assigned territory as need
