Job Description:

We are looking for Adult GeriatricNurse Practitionersto provide primary care services as part of our primary care team. The AGNP will work with a multi-disciplinary team to care for and provide medical services to patients, including examining/conducting tests; analyze records, reports, and test results; to prevent and/or diagnose illness, disease or injury; prescribe/administer treatment, therapy, medication, vaccination, and other specialized medical care to treat or prevent illness, disease, and/or injury; explain procedures and test results on prescribed treatment plan; manage labs, referrals. The AGNP will collaborate with other physician assistants, nurse practitioners, physicians, and other support staff as part of the clinical team. Perform other job-related duties as required by departmental needs/demands.

Experience and Skills:

• Completion of an approved/accredited educational program for Nurse Practitioners

• Valid NY State license, CDS, DEA, and BLS

• Ability to effectively communicate, follow established procedures and protocols

• Understanding of and passion for Community Healthcare is preferred
