Core Duties of a Home Health Occupational Therapist

1. Patient Evaluation

  • Perform comprehensive in‑home assessments of mobility, ADLs, cognition, safety, and functional limitations.
  • Identify barriers in the home environment (stairs, clutter, lighting, bathroom setup).
  • Establish measurable, patient‑centered goals based on functional needs.

2. ADL & IADL Training

  • Teach or retrain skills like bathing, dressing, grooming, toileting, meal prep, and medication management.
  • Modify tasks to match patient ability and promote independence.

3. Home Safety & Environmental Modification

  • Recommend safety equipment (grab bars, shower chairs, raised toilet seats).
  • Reorganize home layout to reduce fall risk and improve accessibility.
  • Provide caregiver training on safe mobility and transfers.

4. Therapeutic Interventions

  • Strengthening, endurance, and fine‑motor training.
  • Cognitive retraining for memory, sequencing, and problem‑solving.
  • Pain management strategies and energy‑conservation techniques.

5. Adaptive Equipment & DME Training

  • Assess need for devices such as reachers, sock aids, walkers, or splints.
  • Train patients and caregivers on safe, effective use.

6. Care Coordination

  • Communicate with RNs, PTs, SLPs, social workers, and physicians.
  • Participate in case conferences and update care plans as needed.

7. Documentation & Compliance

  • Complete OASIS‑aligned documentation (evals, daily notes, progress notes, discharge summaries).
  • Ensure compliance with Medicare and agency standards.
  • Track progress toward goals and justify medical necessity.

8. Patient & Caregiver Education

  • Teach safe mobility, fall prevention, home exercise programs, and compensatory strategies.
  • Provide emotional support and coaching to increase confidence and independence.

9. Discharge Planning

  • Determine readiness for discharge based on functional progress.
  • Recommend ongoing outpatient therapy, community resources, or equipment as needed.
