Key Responsibilities:
- Conduct routine skin assessments and weekly wound evaluations, initiate interventions in collaboration with physicians or wound specialists.
- Maintain weekly wound documentation.
- Reviews admission assessment and validate admission skin assessment.
- Administer medications and treatments as prescribed; maintain accurate documentation in Electronic Health Records.
- Supervise and direct nursing staff; ensure compliance with care plans, MDS requirements, and regulatory standards.
- Communicate effectively with residents, families, and healthcare providers regarding care plans, changes in condition, and discharge planning.
- Perform clinical procedures including venipuncture, IV therapy, and emergency care as needed.
- Oversee admissions, discharges, and transfers; maintain medication inventory and cleanliness of medication carts.
- Participate in interdisciplinary care planning and coordinate wellness activities to support resident health and socialization.
Required Qualifications:
- Associate Degree in Nursing. Licensed as a Registered Nurse in the state of South Carolina.
- CPR Certified.
- Intermediate computer skills.
Preferred Requirements:
- BSN or MSN. Five years of wound care experience in long-term care. Wound Care Certification.
- Advanced computer skills. Knowledge of Electronic Medical Record Documentation and wound staging and compliance.
