Job Summary:
Serves as a member of the multidisciplinary care team, responsible for coordinating the individualized care of stroke patients throughout the continuum of care to increase and assure timely access to healthcare services. Functions as a patient advocate, educator, key point of contact and liaison between the patient and other appropriate services. Participates in community education (educational outreach programs, workshops, seminars, support services).
Job Responsibilities:
1. Navigates stroke patients through the continuum and assists to eliminate barriers to healthcare and enhance the patient experience.
2. Serves as a member of the multidisciplinary care team, functioning as a patient advocate, educator, point of contact and liaison between the patient and all other appropriate services.
3. Reviews stroke patient care daily at Hartford Hospital via documentation review. Works with nursing and provider staff to ensure Joint Commission and American Heart Association standards are met and patients are receiving appropriate and efficient care.
4. Assists the Stroke Program Manager by:
· Preparing for Joint Commission recertification.
· Serving as a resource to collaborate with interdisciplinary staff in providing care to stroke patients.
· Facilitating ongoing training and education of the interdisciplinary team.
· Tracking and reporting of stroke performance measures, quality indicators, stroke guidelines, and supporting/executing performance improvement projects related to improving the care and outcomes of stroke patients.
5. Participates in community education outreach programs, workshops, seminars and support services.
6. Assists to decrease in-patient length of stay, readmissions, emergency department visits and left against medical advice rate of stroke patients.
· Bachelor of Science in Nursing or related field
· 5 years clinical nursing experience
· Active Connecticut Registered Nurse licensure
· Certification in specialty area preferred.
· Spanish fluency preferred.
