The Social Worker would work Monday-Friday 7am-3pm or 9am-5pm full-time in Queens, NY.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Supervise a team of Case Managers.
- Hold regular, structured supervision meetings with direct reports.
- Promote effective case management strategies using Housing First, Harm Reduction, Crisis Prevention Institute (CPI), Motivational Interviewing, and trauma-informed care models.
- Develop and implement individual service plans, ensuring comprehensive and timely documentation of client contact and progress.
- Conduct interviews and assessments of potential clients.
- Provide clinical direction and leadership to the program, assisting with troubleshooting, problem-solving, and strategizing solutions for client engagement and challenging behaviors.
- Foster a staff and resident culture that emphasizes housing placement as a key indicator of success, ensuring clients are prepared for housing placement.
- Oversee the development and implementation of individual service plans, ensuring thorough and timely documentation of client interactions and progress.
- Assess and evaluate client functioning.
- Supervise and facilitate case conferences.
- Coordinate care delivery with multiple service providers, particularly outreach teams.
- Collaborate with outreach teams to ensure individuals succeed in transitional housing and prevent a return to homelessness.
