How you'll make an impact in this role
Architect personalized care journeys by integrating patient values and clinical needs into a seamless transition plan from admission through discharge.
Empower vulnerable populations by providing expert intervention and resources for complex needs, including crisis support, substance abuse recovery, and legal advocacy.
Eliminate barriers to recovery by proactively advocating for community resources and insurance coverage to ensure every patient has what they need to thrive at home.
Lead the interdisciplinary care team through continuous dialogue and collaboration, ensuring the health plan evolves alongside the patient's progress.
Champion clinical accuracy by serving as the expert resource for medical documentation and post-discharge service levels, ensuring care is both safe and appropriate.
