Responsibilities:
The Social Services Designee - Discharge Planner is responsible for assisting and coordinating with the facility's Social Service Department to streamline the process for discharge planning. Reports to the Social Services Director
- Meeting the resident/guest on or shortly after the admission and following them until they discharge to the community.
- Attend the 72 hour care plan meeting.
- Ordering equipment for the community setting.
- Arranging home health nursing for after discharge.
- Coordinating the medications for community use.
- Scheduling IV home services if necessary.
- Communicate with external case managers, as needed.
- Coordinate the entire discharge, with IDT involvement.
- Attending care conferences for sub-acute residents/guests.
- Develop an individualized discharge care plan for the resident/guest discharge in collaboration with the facility, the family, and the practitioner, assuring the resident/guest receives the care that will meet their needs in the community.
Education and/or Experience:
High school diploma/GED
