Position SummaryPrimary emphasis on supporting Senior Services meals on wheels operations, i.e., scheduling, securing volunteers, liaison with food vendor) along with backup duties at congregate dining site and case management team as needed. Includes routine clerical and data entry duties assisting Senior Services staff as needed.
Essential Functions- Support all phases of the home delivered meals, i.e., meals on wheels program to include meal orders, volunteer assignments, and related tasks to assure a smooth delivery on a weekly basis.
- Substitutes for volunteers on other delivery routes when needed.
- Enter accurate program data and statistical information relating to units of service into My Senior Center (MSC), Client information and Registration Tracking System (eCIRTS) and other systems in a timely matter.
- Provide related referrals to case managers for action in serving the seniors’ needs. Weekly referrals will be necessary as they relate to meals on wheels delivery and volunteer feedback, i.e., no delivery, need for delivery, upcoming medical hospitalization/rehab.
- Successful completion of 701B assessment training for certification under the Florida Department of Elder Affairs.
- Completes home visits for client assessments and visits homebound clients in support of case manager as needed. File documentation, inclusive of related file narratives to be completed in monitor-ready condition.
- Covers Flagler County Senior Center operation as assigned to include all related duties.
- Conduct interviews and compliance requirements related to the Emergency Home Energy Assistance Program (EHEAP) to include office and home interviews/visits. Maintain client files in monitor-ready condition to meet random monthly file reviews via ElderSource.
- Coordinate with Senior Service staff in preparation of a monthly Senior Center calendar and its copying and distribution.
- Support distribution of and document approved Nutrition Education materials as outlined monthly by the Nutrition Consultant and/or Program Manager. Presentations at Flagler County Senior Center will be a minimum of 15 minutes to meet compliance standards.
- Data entry of case manager billable hours to eCIRTS at the end of each month for state general revenue reimbursement.
- eCIRTS data entry of Home Care for the Elderly (HCE) as presented by case managers by the specified deadline each month. Task is time critical to allow for monthly reimbursement to caregivers in timely/predictable manner via ElderSource.
- Maintains client confidentiality, including handling of client files according to HIPAA requirements and shredding of sensitive client documents as prescribed by organizational policy.
- Maintains volunteer reporting requirements for completion of required reports and related distribution.
- Serves as courier for division as required.
- Participates in all trainings presented by the Flagler County Emergency Operations Center relating to disaster and/or hurricane trainings.
- Attends staff meetings, workshops, seminars, conference, and trainings as required.
- Performs related work as required.
- During emergencies, all employees are considered essential personnel and may be required to perform alternate duties.

Minimum Qualifications- High School Diploma or equivalent.
- Must have work history in a detailed-oriented position.
- Must possess a valid State of Florida Driver’s License.
