Position Title: RN/LPN Resident Care Manager

Department: Briarwood Health Center

Schedule: $36-$56 DOE/years of experience

Full-Time M-F 8AM-4:30PM (on call weekends)

WHO YOU ARE:

  • Current RN or LPN license issued by the State of Washington
  • Ability to understand and follow instructions in English. Ability to communicate effectively.
  • At least 6 months experience, geriatric experience preferable.

GENERAL SUMMARY:

The Resident Care Manager (RCM) is an RN/LPN who is responsible for the overall nursing care on their shift, including the accurate and timely administration of medications, treatments, documentation and emergency calls.

ESSENTIAL JOB DUTIES:

  • Supervises direct care of residents and nursing personnel.
  • Administers direct care as necessary.
  • Gives and receives shift reports
  • Rounds each shift to assure proper care is rendered in a safe and clean environment.  Provides proper follow-up on MD, psychologist and pharmacy recommendations.
  • Keep physicians and families aware of changes in resident conditions
  • Assists in developing comprehensive care plans, responsible for daily updates to resident care plan from physician orders daily and in-servicing staff on changes to resident care. 
  • Directs and participates in resident move in and move out to include paperwork, ensuring completion of voiding diaries and summary, discharge summary per policy.
  • Maintain accurate documentation of all care rendered; completes quarterly assessments (fall, Braden, dehydration, bowel and bladder, pain physical restraints.)
  • Responds to resident emergency call system
  • Interfaces with other disciplines to ensure continuity of care and services; attends weekly RISK meetings, leads weekly skin rounds and makes necessary changes to promote would healing, attends weekly Medicare meeting and provides a comprehensive weekly note on all Medicare residents for their medical record.
  • Assists with Medical Director/ARNP follow up; rounds as necessary.
  • Encourages restorative care in order to obtain maximum independence; make a comprehensive restorative note in residents medical record monthly.

OTHER DUTIES:

  • Attends Morning Meeting daily and ensures follow up on findings is completed.
  • Follow up on resident lab results from the previous day and take action per policy.
  • Manage the resident weigh program to include weekly and monthly weights per RD recommendation, weight summaries as indicated by Health Center policy.
  • Assures proper functioning of equipment and adequate supplies.
  • Attends in-service training and education sessions as assigned.
  • Performs specific work duties and responsibilities as assigned.

PHYSICAL REQUIREMENTS/WORKING CONDITIONS:

May be required to work up to two weekends per month.  Frequent standing, stooping, bending, stretching, squatting; must be able to stand/walk for up to 70% of the workday; must be able to lift, transfer, turn and position residents weighing up to 125lbs unassisted, or with the assistance of one for a resident weighing over 125 lbs.  May be subjected to offensive odors and combative behavior.  Must be able to lift and carry up to 20 lbs. frequently, and 50 lbs. occasionally.  May be exposed to communicable diseases including the HIV and HBV viruses.  Must be able to work paying close attention to detail with frequent interruptions.  Must be able to see, hear, and communicate effectively with residents and staff.
