Hospice helps patients and their families prepare physically, emotionally and spiritually for the end of life. Our focus is on providing comfort, not cure; on family, not just the patient; and on quality of life, not duration. Cleveland Clinic’s hospice team includes physicians certified in hospice and palliative medicine, nurses, social workers, chaplains, home health aides and volunteers. This team works together to support patients wherever they call home. This position will support our patients living throughout the Cleveland area – from west Cleveland to Vermillion.
A caregiver in this position works 7:00am to 6:00pm or 8:00am to 5:00pm with 1 winter and 1 summer holiday on rotation. Weekend rotations occur every 10 weekends.
A caregiver who excels in this role will:
· Provide supervision, assessment, planning and direct patient care to hospice patients.
· Perform accurate and complete admissions assessments and re-assessments.
· Assess patients for safety risks, abuse and neglect.
· Assess, document and manage patient’s pain.
· Prioritize and document patient problems.
· Administer medications and treatments safely and accurately with a focus on patient comfort.
· Coordinate care for managing a caseload of patients.
· Document changes in patient condition and effectiveness and timeliness of care/interventions.
· Evaluate, update and revise plans of care.
· Assess educational needs of patients, caregivers and family while considering cultural issues and other special challenges.
· Provide teaching and educational material to patients and families concerning diagnosis, diagnostic tests and procedures, safe and proper use of equipment, reason for and safe use of medication food/drug interactions, injury prevention, self-care and nutritional needs.
Minimum qualifications for the ideal future caregiver include:
· Graduate from an accredited school of nursing.
· Current licensure as a Registered Nurse (RN) in the State of Ohio.
· Basic Life Support (BLS) certification from the American Heart Association (AHA) or the American Red Cross.
· One year of current clinical experience as an RN.
· One year of experience in an acute medical-surgical or critical care setting.
· Valid state driver’s license.
· Proof of automobile insurance with $100,000/$300,000 coverage.
Preferred qualifications for the ideal future caregiver include:
· Bachelor of Science in Nursing (BSN).
· Two years of experience
· Able to manage disease progression in an acute setting

Pay Range
Minimum Annual Salary: $73,840.00
Maximum Annual Salary: $115,648.00
