How you'll make an impact in this role

  • Lead the Care Journey:You will implement and continuously monitor individualized patient care plans, adjusting strategies based on real-time observation to ensure the highest quality of care and optimal outcomes.
  • Essential Communication:Accurately record, track, and effectively communicate changes in patient condition and progress to the entire interdisciplinary team, ensuring proactive and timely interventions.
  • Precision and Execution:Flawlessly note and carry out physician and advanced practice provider orders, maintaining adherence to clinical protocols and playing a critical role in the delivery of treatment.
  • Interdisciplinary Orchestration:Act as the primary coordinator for all clinical disciplines—from physical therapy to social work and pharmacy—to ensure seamless, cohesive, and collaborative patient care.
  • Champion of Transition:Proactively assess and coordinate all necessary discharge planning with the patient, family, and healthcare team, setting our patients up for a safe, successful transition and continued wellness outside of our walls.

What minimum qualifications you'll need

Licensure / Certification / Registration:

  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
  • Registered Nurse credentialed from the Oklahoma Board of Nursing obtained prior to hire date or job transfer date required.

Education:

  • Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

What additional requirements you'll need

  • Preferred 5 plus years of L&D experience.
