Job Duties:

A $5,000 hiring bonus is being offered for this position. Hiring bonuses shall only be for new hires of original employment or reemployment as defined in Human Resources Policy No. 5.0.

· $1,250 will be awarded in the first paycheck.

· $1,750 will be awarded upon completion of the six (6) month interim performance evaluation with an overall rating of “Meets Expectations”.

· $2,000 will be awarded upon completion of the employee’s one (1) year probationary period and a performance evaluation with an overall rating of “Meets Expectations”.

The City of Virginia Beach Department of Human Services is eagerly seeking to hire a Registered Nurse Coordinator who has experience working with adults in a behavioral health inpatient or outpatient setting for the Adult Outpatient Services program in the Behavioral Health Division.

Duties:

  • Provide care coordination for adults receiving medication management services for mental health, substance use, or co-occurring disorders in an outpatient setting.
  • Assess needs, coordinate, and complete documentation required for internal referrals including follow up communication with referral sources to maintain continuity of care.
  • Review incoming agency referrals to Medication Management for medical necessity.
  • Facilitate linkage to internal and external services from medical providers, behavioral health programs, housing supports, substance use treatment, jail resources, and benefit resources.
  • Assist clients without a primary care provider in accessing medical care and review medical records at least annually or more frequently for individuals with complex and/or changing medical conditions.
  • Navigate complex healthcare and insurance systems to connect high-risk clients with essential services and providers.
  • Linking individuals to internal and community resources.
  • Maintain clinical records in accordance with regulatory standards and agency policies, including obtaining required client/guardian signatures.
  • Treatment Plan development and updates at least annually in collaboration with clients, prescribers, and treatment team members.
  • Organize treatment team meetings for annual reviews and follow up on level-of-care recommendations from the treatment team.
  • Promote care integration across service providers to ensure comprehensive, person-centered support, and actively supports client engagement in services.
  • Update health records, and supporting discharge planning.
  • Provide coverage for nursing services in the adult outpatient clinic and to other Behavioral Health programs as assigned.

Minimum Requirements:

MINIMUMS: Requires current and valid licensure as a Registered Nurse by the State of Virginia or as a part of the Compact Designation (Multi-State Privileges) and 2 or more years Registered Nurse experience to be specified by the hiring department. Current CPR certification. Must have, or obtain, a current and valid Virginia or North Carolina drivers license in accordance with Virginia or North Carolina DMV drivers license eligibility requirements.

SPECIAL: All employees may be expected to work hours in excess of their normally scheduled hours in response to short-term department needs and/or City-wide emergencies. This position may require that incumbents to wear and maintain appropriate personal protective equipment such as, but not limited to, safety footwear, hard hats, safety eyewear, gloves, or other safety attire and equipment in designated areas of risk. Specific requirements will be determined and communicated by the employees supervisor based on position assignments.

Preferences:

Two (2) years of experience as a Registered Nurse working with adults in a behavioral health inpatient or outpatient setting.
Two (2) years of experience as a Registered Nurse administering psychiatric medications to adults with severe mental illness and substance use disorders.
Two (2) or more years of experience as a Registered nurse documenting behavioral health services in an electronic health record.
Two (2) years of experience completing psychiatric nursing assessments.
Experience using Microsoft Office Products (Excel, Word, Outlook).
Experience performing nursing procedures (e.g., venipuncture, blood pressure, intramuscular injections).
Experience providing care management to adults in a behavioral health setting.
