• Interdisciplinary Coordination: Lead the development and execution of a comprehensive care plan from admission to discharge, ensuring all healthcare providers are aligned on the patient’s goals.
• Patient-Centered Planning: Design care strategies across the continuum that integrate the specific preferences, values, and long-term needs of the patient and their family.
• Advocacy & Barrier Removal: Proactively monitor patient progress through clinical assessments to identify and eliminate obstacles to care or resource gaps.
• Clinical Documentation & Resource Management: Serve as a technical resource for medical record accuracy, ensuring that all services and post-discharge options align with the patient's diagnosis and treatment plan.

Licensure / Certification / Registration:

• Licensed Registered Nurse credentialed from the Florida Board of Nursing obtained prior to hire date
  or job transfer date required. Licensed as a Registered Nurse in Florida required. Credentialed from State Board of
  Nursing or current home state license for multi-state license recognition "Compact State" obtained
  prior to hire date or job transfer date required.
• BLS Provider preferred. American Heart Association or American Red Cross accepted.

Education:

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of
  hire.

• Previous case management experience
