Caritas Family Solutions, formerly known as Human Support Services, is seeking a kind and patient individual to serve as a Qualified Intellectual Disabilities Professional (QIDP). The QIDP is responsible for developing and overseeing the implementation of the Individual Support Plan for 24hr CILA or HBS individuals. This position works collaboratively across agency programs and with external service providers to promote the delivery of quality services to people we serve. The QIDP should be creative in developing strategies and supports that result in people with disabilities achieving personal life goals. Person Centered Plans are developed based on CQLs Personal Outcome Measures and other person centered planning tools.


1. Meet with the Clinical Team weekly for administrative and planning meetings. Seek authority for changes in Programs prior to implementation. Keep supervisor informed of all activities of the Program.


2. Schedule all clinical activities to meet treatment plans of the consumers so as to conduct clinical aspects of the Program to meet the regulations.


3. Help any consumer to participate in the design of the array of services tailored to his or her needs.


4. Work with the supervisor and Residential Manager to develop a community array of services to meet consumer’s needs.


5. Provide informational, educational, and advocacy services to family members and others.


6. Be responsible for clinical records so that all materials are in place and audited on a monthly and quarterly basis.


7. Identify gaps in the service array and report to supervisor.


8. Advocate for consumer rights and services.


9. Provide written records of team meetings and other meetings.


10. See to it that intensive services are in place for consumers of the Residential Program as needed.

TRAINING AND JOB REQUIREMENTS:

  • Must meet criteria set by IDHS to be a Qualified Intellectual Disabilities Professional (QIDP) IDHS: Required Experience, Job & Educational Requirements
  • Bachelors Degree PLUS at least one year of direct work experience
  • Meet with CILA Site Manger monthly at scheduled site meetings to assist with consumer monitoring and planning
  • Attend full CSN trainings quarterly to stay updated on consumer care and/or changes affecting CSN and Human Support Services
  • Successfully complete all required certifications of CSN, HSS and the State of Illinois that relate to your position as scheduled
  • Attend trainings outside required monthly trainings that relate to your position as mutually agreed upon

PHYSICAL REQUIREMENTS:

  • Must have valid driver’s license
  • Have adequate transportation to be able to move from site to site
  • Able to lift 50 pounds and walk flights of stairs
  • Have normal or corrective eyewear and hearing devices
  • Have clarity of speech to be understood

OTHER:

  • Work within agency structure and accept supervision
  • Work constructively using team approach with peers
  • Work with providers in the community
  • Must feel confident to report abuse and neglect of consumers/staff to supervisor
  • Must be flexible and open-minded
  • Do other duties as assigned and/or as mutually agreed upon
