POSITION SUMMARY: These positions manage the day-to-day Rehabilitation or Residential Rehabilitation programming and service delivery in a designated county, including supervision of direct care staff and interns. The two managers lead their staff to come together as a cohesive team to meet the service needs of the consumers they jointly serve. Responsibilities include implementing policies, procedures, regulatory and accreditation standards as well as supporting and monitoring Program Performance Outcomes and other quality assurance measures and activities.
ESSENTIAL DUTIES:
Direct and oversee program operations, ensuring compliance with policies, procedures, fiscal guidelines, regulatory and accreditation standards.
Oversee the day to day provision of services to consumers to support goal achievement, including but not limited to scheduling, programming, and service delivery.
Schedule, supervise and evaluate assigned staff in accordance with policy, ensuring staff accountability for job performance.
Assist staff with managing day to day consumer issues.
Approve service notes daily according to regulatory guidelines and Crossroads protocols; process for billing.
Facilitate consumers’ entry, orientation and transitions.
Ensure clinical and Health Home collaboration for consumers’ care.
Perform assigned quality assurance functions related to compliance and health and safety.
Assume responsibility for healthy and safe environments.
Meet agency productivity measures specific to the position and assigned program.
Operate program within assigned budget parameters.
Requirements
EDUCATION & EXPERIENCE REQUIREMENTS: AA or BA in Behavioral Health, Psychology, Social Work or related field. Minimum 1 year (with BA) or 3 years (with AA) positively referenced experience in the field. Supervisory experience strongly preferred. Must be or agree to become a Certified Psychiatric Rehabilitation Practitioner through the Psychiatric Rehabilitation Association (PRA) within 2 years.
SCHEDULE: Flexible based on consumer and program needs.

ADDITIONAL DUTIES & RESPONSIBILITIES – SPECIFIC TO POSITION
- New Consumers:
- Complete and distribute Consumer Action Forms, as needed
- Maintain RTM calendars and ensure that RTMs occur timely.
- Develop Crisis Management and Managed Intervention Plans and submit for approval.
- Ensure communication and staff resources to support consumers' compliance with appointments.
- Draft communication to consumers as needed - i. e. outreach, discharge warning, etc.
- Create and submit consumer Discharge Summaries
- Ensure continuation of service delivery to consumers during direct care staff absences by providing services, as necessary.
- Review Representative Payee expenditures monthly and process checks.
- Develop and distribute monthly calendars for onsite programming specific to youth, young adult, and adult populations. (PRP Manager)
- Facilitate monthly Members’ Forum with consumers. (PRP Manager)
- Facilitate elections and functions of consumer Executive Committee. (PRP Manager)
- Coordinate van runs for Club attendance. (PRP Manager)
- Communicate and coordinate with inpatient facilities as needed. (RRP Manager)
- Monitor and ensure consumer medication changes are carried out properly by staff. (RRP Manager)
- Ensure that assigned facilities, furnishings and supplies are maintained to meet health & safety and regulatory standards.
- Perform health and safety inspections for assigned sites, provide reports, and follow-up action.
- Communicate promptly vehicle and facility repair/replacement needs according to agency procedure.
- Inspect agency vehicles assigned to the program monthly, at a minimum, and ensure cleanliness. (PRP Manager)
- Review and submit agency vehicle mileage logs monthly. (PRP Manager)
- Oversee and track service contacts, assessment, plan and monthly summary note completion.
- Oversee Child & Adult Care Food Program (CACFP): consumer enrollment forms, menu approval, reporting. (PRP Manager)
- Facilitate CQT visits (PRP Manager)
- Review MMR’s weekly for accuracy and compliance (RRP Manager)
- Review House Books monthly for compliance. (RRP Manager)
- Assign, monitor, and maintain case load lists.
- Lead weekly team meeting, document and distribute team meeting minutes
- Complete and submit End of Month paperwork/reports
- Coordinate and communicate with approved vendors regarding facility repairs.
- Process monthly Cost of Care payments and forward to Finance Department. (RRP Manager)
- Review Residential Expense Sheets weekly (RRP Manager)
- Maintain petty cash account for designated county, as assigned.
- Provide back-up support to Coordinator for initial screening interviews.
