Position Overview
We are seeking a Primary Care Physician (MD/DO) to join a growing, multi-site outpatient medical group serving communities across Brooklyn and greater NYC. This is an excellent opportunity to work within a collaborative, team-based environment that integrates primary care with a wide range of specialties.
This role is ideal for physicians who thrive in a fast-paced outpatient setting, enjoy building long-term patient relationships, and are open to a dynamic care model that may include telehealth and home visits.
Practice Environment
Multi-site outpatient network across Brooklyn
Locations may include: Clinton Hill, Williamsburg, Park Slope, Prospect Heights, East Flatbush
Team-based model with MDs, NPs, PAs, and specialists
Integrated services including diagnostics, imaging, and specialty care
Centralized administrative and clinical support
Patient Population
Diverse NYC patient base
Adults and geriatric patients (some pediatrics depending on site)
Mix of Medicare, Medicaid, and managed care
Emphasis on preventive care and long-term patient management
Compensation & Benefits
Base Salary: $215k-$250k
Productivity bonus potential (RVU/collections-based)
Full benefits package (health, malpractice, PTO)
Earning potential up to ~$300K+ with productivity
Job Responsibilities
Key Responsibilities
Provide comprehensive outpatient primary care services
Manage acute and chronic conditions (HTN, diabetes, etc.)
Perform preventive care, physicals, and wellness visits
Coordinate care with in-house specialists (cardiology, GI, neurology, etc.)
Maintain accurate and timely EMR documentation
Participate in telehealth visits as needed
Potential involvement in home visits (based on preference and role structure)
Qualifications
Required Qualifications
MD or DO degree
Board Certified or Board Eligible in Internal Medicine or Family Medicine
Active New York medical license (or ability to obtain)
Strong clinical and communication skills
Ability to work in a collaborative, high-volume environment

Full-time outpatient schedule
Flexible structure may include:

Potential evenings/weekends (based on clinic needs)

Strong outpatient primary care clinical skills
