Who You Are
We are seeking an experienced and drivenNurse Practitioner (NP)to serve as ourPractice Clinical Leadin anon-site leadership rolebased in Orangeburg, NY.
This position combinesclinical care, team leadership, operational oversight, and program development. You will play a key role in managing the NP team, ensuring compliance, and driving performance across all clinical programs.
What You'll Do
Key ResponsibilitiesClinical Leadership & Oversight• Provide oversight across CCM, RPM, BHI, and telehealth programs• Supervise and mentor Nurse Practitioners and clinical staff• Ensure compliance with CMS guidelines, HIPAA regulations, and internal standards
Training & Development• Train and onboard new Nurse Practitioners• Provide ongoing coaching and performance management• Foster a high-performance, accountable clinical team
Quality Assurance & Compliance• Conduct audits of documentation and clinical workflows• Ensure timely and accurate charting (within 48 hours)• Maintain audit readiness and regulatory compliance
Program Development & Operations• Develop and refine clinical templates and workflows• Collaborate with operations and field leadership• Support rollout of new programs and technologies
Care Coordination• Communicate care gaps and findings to partner facilities• Work with interdisciplinary teams (LPNs, care coordinators, facility leadership)• Oversee NP-to-facility assignments and team structure
Direct Patient Care• Conduct approximately50 virtual patient visits per week• Provide care across CCM, RPM, and BHI programs• Develop individualized care plans
What You Bring
Qualifications• ActiveNurse Practitioner (NP) license• Willingness to obtain and maintainmulti-state licensure (TX, FL, PA required)• Experience inCCM, RPM, telehealth, or value-based carepreferred• Proven leadership or management experience• Strong communication, documentation, and clinical decision-making skills• Ability to work on-site in Orangeburg, NY

Compensation & Schedule•Salary: $170,000 – $180,000 per year with performance-based bonus opportunities•Schedule:
