Basin Home Health & Hospice is looking for a licensed Physical Therapy Assistant (PTA) to join our home health team on a PRN or part-time basis. This is a pay-per-visit role built for a PTA who wants flexibility, meaningful one-on-one patient relationships, and the kind of clinical autonomy you don't get in a clinic or hospital setting.


You'll work directly with patients across the Farmington area, helping them regain strength, mobility, and independence at home — collaborating with a supervising Physical Therapist and an experienced interdisciplinary care team that has served the Four Corners region for years.


What You'll Do

  • Provide skilled physical therapy treatments in patients' homes under PT supervision
  • Implement care plans and therapeutic interventions as directed
  • Monitor and document patient progress accurately and on time
  • Educate patients and families on home exercise programs, safety, and mobility
  • Assess home environments for safety and care appropriateness
  • Participate in biweekly interdisciplinary episode management meetings
  • Complete documentation in compliance with agency and HIPAA standards


What We're Looking For

Required:

  • Current New Mexico PTA license in good standing
  • Ability to work independently in a home-based care setting
  • Comfort navigating rural and semi-rural patient environments
  • Strong verbal and written communication skills
  • Proficiency with EMR documentation


Schedule & Compensation

  • Primarily Monday–Friday, with flexible daily scheduling based on patient visits
  • Pay-per-visit structure
  • Company vehicles or mileage reimbursement provided
  • 401(k) available
