Physical Therapist Assistant (PTA) – Home Health

Location: Naples, FL

Schedule: Full-Time and Per Diem

Compensation: $35.00 – $38.00 per hour

Overview

An established and growing home health provider is seeking a Physical Therapist Assistant (PTA) to deliver high-quality, one-on-one care in the home setting throughout Naples.

This role is ideal for a clinician who enjoys working closely with patients, values autonomy in the field, and is focused on helping individuals improve mobility, function, and overall quality of life.

What You'll Do

Patient Care

  • Provide in-home physical therapy services under the supervision of a licensed Physical Therapist
  • Execute established treatment plans focused on mobility, strength, balance, and functional independence
  • Monitor patient response to treatment and report progress or concerns to the supervising PT
  • Support patients in achieving optimal recovery and independence

Care Coordination

  • Collaborate with the supervising PT and interdisciplinary care team to ensure continuity of care
  • Participate in case communication and support overall care plan execution
  • Assist with discharge planning and patient transitions

Patient & Family Education

  • Educate patients and caregivers on prescribed exercises, mobility techniques, and safety practices
  • Reinforce treatment plans to promote adherence and long-term success

Documentation & Compliance

  • Complete accurate and timely documentation in accordance with Medicare and home health guidelines
  • Maintain compliance with all regulatory and clinical standards

What We're Looking For

  • Graduate of an accredited Physical Therapist Assistant program
  • Active Florida PTA license/certification
  • Minimum 1+ year of experience (home health experience preferred)
  • Strong clinical judgment and ability to work independently within scope
  • Valid Florida driver's license, reliable transportation, and active auto insurance
  • Strong communication and patient engagement skills
