Job title: Staff Pharmacist - Hospital Pharmacy
Schedule: Full-time, 80 hours per pay period; Monday-Friday, scheduled between 7:00 AM - 4:30 PM
Weekend rotation: Every 4th weekend; 7:00 AM - 3:00 PM
Holiday rotation: Every 4th holiday
Position Summary:
The Staff Pharmacist performs clinical and professional pharmaceutical work in the compounding and dispensing of drugs and IV supplies.
Position Responsibilities:
- Interpret physician's order from direct copy or via electronic provider order entry.
- Coordinate proper scheduling of the administration of medication with nursing service.
- Provide clinical drug information to medical and nursing staff.
- Maintain patient profiles and monitor patient medications.
- Make decisions in selection of drugs and use of drugs including use of formulary and formulary substitutions.
- Participate in inservice activities (teaching).
- Compound prescriptions and IV admixtures. Supervises supportive personnel.
- Dispense prescriptions using acceptable and good pharmaceutical technique.
- Responsible for following departmental policies and procedures.
- Supervises interns, externs, pharmacy technicians, and other support personnel in the absence of or as designated by the director.
- Actively participate in the Antibiotics Stewardship program
- Maintains a working knowledge of the MBMC antibiogram.
- Maintains a working knowledge of the spectrum of activity of formulary antibiotics.
- Work to affirm the appropriate use of antibiotics in all situations.
- Manage antibiotic de-escalation - IV to PO, C&S response, etc.
- Lead the antibiotic pause.
- Lead efforts to reach goals of therapy for each patient we work with.
- Performs other related duties as required including computer database maintenance for pharmacy systems and automated dispensing units, pump library maintenance and records of activities.
- Actively provides clinical pharmacy services to the patients of Mile Bluff Medical Center
- Renal dosing services.
- Pharmacokinetic services.
- Monitoring of the effect of medications including reviewing and responding to affected lab values, reviewing and managing ADR's, etc.
- Provides medication reconciliation at the time of admission, transfer, and discharge as appropriate for each patient.
- Provides medication education to patients, caregivers, and professional staff.
- Participates in the department's DUE activities with data collection when needed.
- Coordinates the Formulary file updating in the pharmacy computer system and the facility's drug database on an ongoing basis.
- Supervises and trains intern-pharmacists.
- Participates in in-service training.
Position Requirements:
- Professional Pharmacy degree required. Doctorate preferred.
- Must hold and maintain current Wisconsin Pharmacist license.
- BLS certification required.
- 1+ years related work experience required.
- Exceptional accuracy and attention to detail required.

- Strong quantitative and analytical competency.
- Familiarity with pharmacy law.
