Day ShiftDescription:The Manager of the Patient Navigation Department will have direct and significant influence on defining and implementing value-based care network navigation and population health care coordination strategies and initiatives of Saint Alphonsus Health System (SAHS) and the Saint Alphonsus Health Alliance (SAHA) and its participating providers.
This is a HYBRID position.
Minimum Qualifications
Licensed in the state of Idaho as a Health Care Professional (i.e. RN/LPN)
Licensed in the state of Oregon as a Health Care Professional (ie RN/LPN) within 6 months of hire.
Bachelor’s degree in healthcare related field or 4 years of applicable experience in lieu of degree.
10 years of clinical experience, ambulatory care setting preferred
Previous experience with health networks, insurance companies and/or Third-Party Administrators preferred.
The Manager of the Patient Navigation Department is responsible for:
Development and implementation of a comprehensive, multi-disciplinary value-based care network navigation model which positions SAHS/SAHA network to deliver upon Advance Payment Model (APM) insurance contracts and ensure patients receive coordinated, high-quality, and cost-effective care that improves outcomes while reducing unnecessary utilization.
Interfacing and collaboration with leadership and a broad network of employed and independent physician practices, hospitals, clinics, post-acute providers, and ancillary providers to facilitate effective network navigation and coordination for assigned covered lives.
Ensuring programs are designed to support the patient, encourage appropriate resource utilization and facilitate effective stewardship.
Collaboration with Saint Alphonsus Medical Group (SAMG) leaders and colleagues to ensure that case/care/navigation/population health management activities are aligned, complimentary and not duplicative.
Assisting in the development of direct-to-employer network navigation strategies including, patient outreach and care coordination.
Working closely with identified employer groups, Third Party Administrators (TPAs) and insurance companies to develop and execute comprehensive patient navigation/care coordination strategies including tracking and reporting applicable data.
Providing leadership, management, direction, and support to all assigned colleagues.
