Job Title: Advanced Practice Clinician (APC) – Nurse Practitioner
Location: Carroll and Galaxy City counties in Virginia
Duration: 1 year contract (Potential Hire) | Full-Time- 40hrs a week
Schedule: Monday–Friday, 8:00 AM – 5:00 PM; No on-call requirements, no weekends, no holidays

Overview:
The APC plays a critical role as part of an interdisciplinary care team, delivering comprehensive, patient-centered care to complex populations.
Primary Responsibilities:
- Conduct comprehensive medical and behavioral health assessments
- Manage acute, chronic, and complex medical conditions
- Provide advanced illness management and advanced care planning discussions
- Educate and counsel patients and caregivers on health conditions and care plans
- Document accurate and complete clinical notes, including ICD-10 coding
- Collaborate effectively with an interdisciplinary care team
- Ensure continuity of care for high-risk geriatric and chronically ill patients
Required Qualifications:
- Active, unrestricted NP or PA license in the state of residence or ability to obtain by start date
- Education:
- NP: Graduate of an accredited Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) program
- PA: Graduate of an accredited Physician Assistant program
- Board Certification:
- NP: AANP or ANCC certification (FNP, ANP, GNP, AGACNP accepted)
- PA: NCCPA certification
- Current, active DEA license/prescriptive authority
- Ability to obtain a collaborative practice agreement if required by state law
- Full COVID-19 vaccination required (proof prior to employment)
- Reliable transportation
Physical & Travel Requirements:
- Ability to lift and transport a 30-pound bag
- Navigate stairs and various facility and residential environments
- Ability to sit, stand, kneel, and perform physical assessments
- Travel during shift between facilities
- Coverage of 2 facilities:
- Hillsville Health & Rehab Center
- Waddell Nursing and Rehab Center
- Service areas include Carroll County and Galax City, Virginia
Preferred Qualifications:
- 1+ year of clinical experience, preferably in long-term care
- Experience managing patients with complex behavioral, social, and functional needs
- Strong understanding of geriatrics, chronic illness, and end-of-life care
- Proficiency with electronic medical records and clinical documentation
