Nurse Practitioner — Adult/Family/Geriatrics

Morris Heights Health Center is seeking a dedicated and experienced Nurse Practitioner to join our Adult, Family Medicine, and Geriatrics care teams.

This is an excellent opportunity for a mission-driven provider who is passionate about delivering high-quality, comprehensive primary care in a community health setting. The Nurse Practitioner will play a key role in expanding access, strengthening chronic disease management, supporting preventive care, and improving health outcomes for patients across the adult lifespan, including older adults with complex care needs.

Key Responsibilities

The Nurse Practitioner will:

• Provide comprehensive primary care services to adult, family medicine, and geriatric patients.
• Conduct health assessments, physical examinations, diagnosis, treatment planning, and follow-up care.
• Manage acute and chronic conditions, including hypertension, diabetes, asthma/COPD, cardiovascular risk, obesity, behavioral health comorbidities, and age related health concerns.
• Order, interpret, and follow up on diagnostic tests, labs, screenings, and referrals.
• Support preventive health initiatives, including cancer screenings, immunizations, wellness visits, and risk assessments.
• Collaborate with physicians, nursing, behavioral health, care management, social services, and enabling services to support whole-person care.
• Maintain timely, accurate, and complete documentation in the electronic health record.
• Participate in quality improvement efforts tied to access, productivity, patient experience, UDS measures, PCMH standards, and value-based care goals.
• Provide patient education that supports disease prevention, medication adherence, lifestyle modification, and self-management.
• Support care coordination for medically complex patients and patients with social drivers of health needs.

Qualifications

• Current New York State Nurse Practitioner license.
• Board certification as an Adult-Gerontology, Family, or Adult Nurse Practitioner.
• Active DEA registration or eligibility to obtain.
• Current BLS certification.
• Experience in primary care, community health, FQHC, adult medicine, family medicine, or geriatrics strongly preferred.
• Strong clinical judgment, communication skills, and commitment to patient-centered care.
• Ability to work effectively in a fast-paced, team-based environment.
• Bilingual ability is a plus.
