Hospice Medical Social Worker - Provide individualized and coordinated hospice care to bring safety and comfort to the patients and families. You will have independence and flexibility in the field to visit patients one on one in their home environment.
Hours: Full-time | Days | Monday - Friday and On Call Rotation

Territory: Our office is in Roseville, MN and the Social Worker will have a general territory within our service area where they will provide in-home hospice care to the patients. The territory for this position would primarily be in the Southern section of the Twin Cities Metro area.
