SUMMARY

The LPN MDS role is responsible for various tasks related to resident assessments and care planning including collecting resident data, collaborating with interdisciplinary teams, and contributing to care planning processes to enhance the quality of resident care.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Conducts required initial, quarterly, annual, and significant change assessments based on Resident Assessment Instruction (RAI) guidelines to ensure compliance with state and federal standards.
  • Collects, analyzes, and documents data about each resident’s health condition and medical history.
  • Develops, revises, and reviews person-centered care plans as necessary.
  • Communicates frequently with residents and their representatives and coordinates care plan meetings.
  • Collaborates and effectively communicates with nursing staff and facility personnel to coordinate quality care for residents.
  • Attends Staff/Unit meetings promoting, developing, and fostering a positive working environment.
  • Functions as an integral part of the Quality Improvement Program and contributes to improving the facility's overall quality of care provided to residents.
  • All other duties as assigned.

SUPERVISORY RESPONSIBILITIES

This position does not have supervisory responsibilities.

QUALIFICATIONS EDUCATION and/or EXPERIENCE

To perform this job successfully, an individual must be able to perform each primary function satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

  • Associate degree or equivalent from a two-year college or technical school and minimum 2 years of experience in the field.
  • Must have prior experience with MDS assessments and care plans.
  • Certified as a Resident Assessment Coordinator.
  • Strong assessment and Clinical skills.
  • Proficient in electronic health record systems and MDS software.
  • Effective communication and interpersonal skills.

LANGUAGE SKILLS

Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the public. Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of the organization.

MATHEMATICAL SKILLS

Ability to work with mathematical concepts and to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.

REASONING ABILITY

Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

CERTIFICATES, LICENSES, REGISTRATIONS

Valid State of Florida LPN License and BLS CPR Certification.
