(ETCIISI) JOB DESCRIPTION Position Title: Licensed Vocational Nurse (LVN) Reports To: Director ofOperations/Performance Improvement Coordinator (DOOIPIC) JOB SUMMARY: Toassist themedical team inaprofessional manner inacommunity health center setting. Toadminister quality nursing care topatients inaccordance with established nursing standards. Responsibilities include primary health careofpatients ofallages, immunizations andhealth education. ESSENTIAL FUNCTIONS: Pharmacy: 1.Maintains anadequate inventory ofpharmacy medications andprepares medication orders. 2.Assists Pharmacist inre-packaging, re-labeling andlogging ofmedications. 3.Fills medication orders inbackup totheAncillary Services Clerk. 4.Verifies patient’s medications with provider’s orders. 5.Responds topatient andoutside pharmacy calls forrefills andclarification oforders. 6.Responsible forfullpersonal compliance with allapplicable regulations fortheoperation ofaClass D Pharmacy. 7.Uses proper telephone etiquette. 8.Does notmake orreceive excessive personal calls. 9.Ifbilingual, translates inSpanish between non-Spanish speaking staff andSpanish speaking patients as requested. 10.Performs allother duties asassigned. Clinical; inaddition toPharmacy: 11.Assists inproviding quality careinamanner appropriate totheageofthepatient being served according toETCHSI policy andprocedures while providing privacy topatients. 12.Maintains accurate andcomplete records reflecting care rendered; i.e.,patient’s medical record, logs and encounter form. 13.Provides tothepatient andfamily/significant others education about illness andtheplanofcare. 14.Identifies and responds promptly tochanges inpatient’s condition and/or clinic environment. Can independently initiate code procedures andassist asneeded. Demonstrates knowledge oftheVocational Nurse Entitlement Act/Practice Act. 15.Interacts with ETCHSI personnel andassists providers toensure optimal patient care; clarifies orders as needed. 16.Assists withclinical stafftraining andcompetency/skill assessments. 17.Maintains costcontrol ofpatient supplies byeliminating excessive waste. 18.Maintains adequate inventory ofmedical forms, supplies, in-house medications, STD medications and immunizations andcompletes monthly reports asrequired. 19.Performs-delegates themaintenance andcontrols onmedical equipment anddocuments results. 20.Organizes jobduties andtakes initiative inresolving problems/conflicts which enables timely completion ofquality work. 21.Provides apositive rolemodel forstaff andassists inpromoting their professional growth. 22.Becomes andremains familiar with andcorrectly practices allETCHSI anddepartmental policies and procedures. 23.Administers allmedications accurately using thefive(5)rights, documenting medications according to ETCHSI policy andprocedures. 24.Performs thefollowing CLIA-waived labtasks: a.Performs labprocedures asordered byclinic providers. b.Maintains alllablogs; i.e.,STD, pap,refrigerator temperature log,etc., asassigned. c.Maintains aclean, well stocked labarea, reorders supplies, asassigned.Lead LVN forPersonnel, Training, andEquipment; inaddition totheabove duties: 1.Ensures appropriate availability andutilization ofnurses andmedical assistants tomeet patient needs. 2.Assists theDirector ofOperations (DOO) inobserving, analyzing, andrevising patient flow through the clinic process tooptimize traffic patterns, minimize patient waiting time, enhance communications, while maintaining patient confidentiality. 3.Assists nurses andmedical assistants regarding daily operation issues. 4.Makes personnel recommendations totheDOO, including: hiring, employee performance reviews, salary adjustments, promotion, transfer, andtermination ofstaff upon Director ofOperation’s approval. 5.Provides orientation andin-service training fornurses andmedical assistants. 6.Ensures medical equipment isproperly used andmaintained. LVN forFacility, EMR, SIJOs, QA/PLIRM; inaddition totheabove duties: Oversees Medical facility maintenance. vides orientation andin-service training fornurses andmedical assistants intheuseoftheElectronic Medical Record. 9.Assists theDOO, inconjunction with theMedical Director, with thereview and/or updates oftheclinical standing delegation orders andprotocols atleast every twoyears orasneeded. 10.Actively participates inquality assurance/performance improvement/risk management (QA/PIJRM) program through activities including, butnotlimited to: a.assigns datacollection tasks; b.assembles results ofdatacollection forthePTCoordinator; c.conducts/coordinates focused reviews; d.distributes andexplains QA/PI/RM reports toallclinical staff, including Providers. 11.Assists with thecollection andreporting ofdataperfederal, state, andentity requirements. 12.Assists DOO with thereview and/or updates ofthepolicy andprocedures manuals relating toclinical care. Identifies andmeets educational needs forself. 7.Isreceptive toguidance andconstructive criticism. 8.Attends ETCHSI educational programs. 9.Attends andparticipates instaff meetings. 10.Demonstrates awillingness toparticipate andfollow through with special projects andcommittees. 11.Abides byETCHSI attendance policy andestablished procedures fornotification incaseofabsence. 12.Takes initiative inresolving problems andconflicts. 13.Assists inmaintaining asafe, secure andaseptic environment forallpatients andemployees. 14.Adheres toregulatory agency requirements with respect toinfectious disease andhazardous materials. 15.Isfamiliar with andabides byOSHA Standards, Hazardous Materials, Universal Precautions, useof personal protective equipment, Evacuation, Fire, Disaster, andSafety Plans. 16.Uses appropriate body mechanics andfollows safety procedures. 17.Obtains annual TBtestandattends Safety Recertification annually. 18.Completes Incident Reports within 24hours ofincident andturns them intotheSafety Officer.EDUCATION: Licensed Vocational Nurse degree from anaccredited school ofnursing. Must provide proof ofContinuing Education Units (CEU) every twoyears. EXPERIENCE: Oneyearofrelated clinical experience preferred. LICENSURE: Licensed bytheTexas Board ofVocational Nurse Examiners. Maintains certification inCPR. SKILLS: Needs strong communication skills andtheability toaccept responsibility. Utilization ofsound judgment in making decisions isessential. Theability towork cooperatively with community agency personnel to coordinate services forpatients. Advanced skills inmaking assessments ofpersons andsituations accurately andwith clarity. Ability toprepare statistical andadministrative reports. Ability todocument, with clarity, all information relevant toapatient’s needs. Requires theability todistinguish letters orsymbols. Requires the useofvarious pieces ofmedical equipment. Requires theuseofoffice equipment such asbutnotlimited to computers, faxmachines, telephones andcopiers. Bilingual (English/Spanish) aplus. TYPICAL PHYSICAL DEMANDS: Requires fullrange ofbody motion including handling andlifting patients, manual andfinger dexterity and eye/hand coordination. Requires standing andwalking forextensive periods oftime. Requires working under stressful conditions orworking irregular hours. Requires frequent exposure tocommunicable diseases, body fluids, toxic substances, medicinal preparations andother conditions common toaclinic environment.
