Overview Interim HealthCare of Montgomery County is seeking a dependable and compassionate Licensed Practical Nurse (LPN) to provide one-on-one care in a home setting.
This role is ideal for nurses who prefer consistent patient assignments, lower patient ratios, and the ability to build meaningful relationships while delivering skilled nursing care.

Responsibilities - Provide direct skilled nursing care in the patient’s home
- Monitor and document vital signs, patient status, and changes in condition
- Administer medications and treatments as ordered
- Assist with ADLs (as appropriate to care plan)
- Perform trach care, G-tube care, wound care, or other skilled tasks as needed
- Communicate effectively with RN supervisors, physicians, and care team
- Maintain accurate, timely documentation in the EMR
Qualifications - Active LPN license in Tennessee
- Current CPR certification
- Experience in home health, private duty, or long-term care preferred
- Strong clinical and observation skills
- Reliable transportation and ability to travel to assigned case
Preferred Experience - Pediatric or adult ventilator/trach experience
- G-tube and enteral feeding management
- Wound care and chronic condition management
Physical Requirements - Ability to assist with patient mobility and transfers
- Ability to perform extended patient care shifts (8–12 hours typical)
