Job Title: Licensed Practical Nurse (LPN)

Location: Flagstaff, AZ (Onsite)

Duration: 05/11/2026 – 06/30/2026

Schedule: Full-time (40 hours/week) – Day or Night Shift

Job Description:

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide quality nursing care to residents in a long-term care/geriatric setting. The LPN will be responsible for administering medications, performing treatments, monitoring patient conditions, and supporting interdisciplinary care teams to ensure optimal resident outcomes.

Key Responsibilities:

• Administer medications in various forms including oral, enteral, parenteral, and others as prescribed
• Perform nursing treatments such as wound care, tracheostomy care, catheter care, and other procedures within scope of practice
• Monitor, observe, and report patient responses to treatments and medications
• Complete admission assessments and assist with care planning, including Minimum Data Set (MDS) documentation
• Maintain accurate and timely clinical documentation
• Communicate effectively with residents, families, and healthcare team members
• Participate in shift reports, staff meetings, and quality improvement initiatives (QAPI)
• Ensure adequate medical supplies and medication inventory

Required Qualifications:

• Active Arizona LPN License
• CPR/BLS Certification
• Level 1 Fingerprint Clearance Card
• TB Skin Test
• OIG/SAM clearance
• Hepatitis B and COVID-19 vaccination (or approved declination)

Preferred Qualifications:

• Experience in geriatric or long-term care settings
• 2-Step TB Test

Required Skills:

• Knowledge of geriatric nursing practices and pharmacology
• Strong patient observation and assessment skills
• Ability to respond effectively to medical emergencies
• Excellent communication and interpersonal skills
• Ability to work collaboratively within a team environment
• Understanding of federal and state nursing regulations
