Overview:

We are seeking a Licensed Clinical Social Worker (LCSW) to join a collaborative behavioral health team. This role focuses on providing emotional support, therapeutic interventions, and care coordination for individuals and groups, with a strong emphasis on integrated health care and patient wellness.

This is a hybrid opportunity, offering flexibility between in-person and remote work.

Key Responsibilities:

• Conduct client assessments through interviews and personal history reviews.
• Develop and implement individualized treatment plans.
• Evaluate client progress and determine appropriate milestones.
• Provide crisis assessments, triage, and appropriate interventions.
• Facilitate referrals to other healthcare professionals and community programs.
• Deliver therapeutic and wellness-focused services in both individual and group settings.
• Maintain accurate, complete, and timely client documentation.
• Collaborate with interdisciplinary teams to support integrated care.

Qualifications:

• Master’s degree in Social Work.
• Active LCSW license in New York.
• 1–2 years of clinical experience in the mental health field.
• Strong communication, clinical, and organizational skills.
