Primary Care Provider (MD/DO) | Pittsburgh, PA

Full-time | Monday to Friday 8 am to 4:30 pm | PACE Model of Care

The Primary Care Provider (MD/DO) role is designed for physicians who want to step away from volume-driven practice and focus on meaningful, relationship-based care in Pittsburgh.

This opportunity is ideal for a Primary Care Physician, Family Medicine Physician, Internal Medicine Physician, or Geriatrician seeking a smaller panel, longer visits, and true interdisciplinary collaboration.

You will care for older adults in a fully integrated PACE model that prioritizes prevention, continuity, and quality of life over productivity quotas.

What You’ll Do

• Provide comprehensive primary care tailored to each participant
• Manage chronic and acute conditions with a preventive focus
• Collaborate daily with nurses, social workers, therapists, and specialists
• Coordinate care across clinic, hospital, skilled nursing, and home settings
• Participate in shared 1:6 weeknight and weekend phone call coverage

What You’ll Need

• Active Pennsylvania MD or DO license
• Board Eligible or Board Certified in Internal Medicine or Family Medicine
• DEA registration
• Strong communication and interdisciplinary collaboration skills
• Experience in geriatrics or complex care preferred
