Intake Nurse
Description
Position Summary:
Hiring for an Intake Nurse to support an OB clinic as well as a Care Coordination team for 32 hours/week. This role will provide patient education, triage and case management for patients. This role is also responsible for managing patients' medical records, referring to social services and programs available (if applicable), The Patient population of the clinic is diverse and this position will work closely with interpretive services.
Responsibilities:
- Intake Assessments for the OB clinic and the Chronic Disease clinic
- Supporting Care Coordination team
- Supporting Telephonic Triage team that is located on site
Additional Skills & Qualifications
Current Minnesota RN licensure
Current BLS/CPR certification
Prior experience in Intake Assessments strongly preferred
Spanish Speaking is highly preferred

Job Type & LocationThis is a Contract position based out of Minneapolis, MN.
Pay and BenefitsThe pay range for this position is $36.00 - $45.00/hr.
