Title: Clinical Informatics Analyst :: Homecare Homebase (HCHB)

Location: Remote

Duration: 6+ Months, Contract-to-Hire


Role Overview

We are seeking a Clinical Informatics Analyst with strong experience supporting EHR platforms in Home Health and Hospice environments. This is a hands-on role focused on system support, workflow optimization, and data integrity, partnering closely with clinical, operational, and technical teams to improve system performance and usability.


Key Responsibilities

  • Serve as a subject matter expert (SME) for EHR workflows across Home Health and Hospice
  • Troubleshoot and resolve complex system and application issues
  • Support system configuration and optimization aligned with clinical workflows
  • Analyze processes and drive workflow improvements and standardization
  • Collaborate with vendors and internal teams for issue resolution and enhancements
  • Support reporting, data validation, and data quality initiatives
  • Participate in system upgrades, releases, and enhancement projects
  • Ensure HIPAA compliance and proper PHI handling
  • Maintain documentation of workflows and system configurations


Must-Have Skills

  • Strong hands-on Homecare Homebase (HCHB) experience
  • Experience supporting Home Health and/or Hospice workflows
  • Knowledge of OASIS, PDGM, and Hospice processes
  • EHR system support, optimization, and configuration
  • Strong troubleshooting and issue resolution skills
  • Data validation and reporting experience
  • Strong analytical and communication skills


Preferred Skills

  • Clinical background (RN, LPN, or similar)
  • Experience working with EHR vendors and system enhancements
  • Exposure to system upgrades and releases
  • Healthcare data analytics/reporting experience
